Information you provide
The quote form and supplemental collect business and contact details, operations, requested insurance terms, security-control answers, incident information and an applicant attestation. Thrive uses this information to respond to your request, review your application, maintain insurance records and seek available insurance terms. Information may be shared with insurers, wholesale brokers and service providers as needed for those purposes.
Do not enter patient names, medical records, Social Security numbers, passwords or other information that the application does not request. Contact Thrive before sending sensitive supporting documents.
Application processing
Submitted supplementals are stored in a private application record and sent to Thrive’s insurance team with a completed PDF. The website uses hosting, database and email services to support this process. The on-screen application is not a permanent saved draft; refreshing or closing the page may clear unfinished answers.
Campaign and site activity
The site uses browser session storage to remember campaign labels and a random visit identifier within a browsing session. It records visits to selected pages, application starts and completed steps to understand how visitors reach the application. Campaign information may be attached to a submitted application and the corresponding CRM lead.
These activity records contain page paths, campaign labels, the referring website’s hostname and the random identifier. They do not contain application answers, contact details or full referring URLs. The activity endpoint uses a daily-changing hash of the connection address for rate limiting; it does not store the raw address in the activity table. Hosting and security providers may separately maintain operational logs. Funnel activity reporting is skipped when the browser sends a Do Not Track value of “1.”
